Well, going by recent results...
The back five is perfect. But Lampard's days of being a regular starter are over. Replace him with Ramires.
So that leaves Mata on the right, Oscar in the centre and Hazard on the left.
That's preety much perfect. Unless a Chelsea fan wants to correct me.

Considering the last few games I think Lampard will not starting.
I think the formation will:
With Ramires, Mikel, Hazard over Lampard

Short Free Kick: Juan Mata and occasionally David Luiz.
Long Free Kick: David Luiz
Free Kick 2: ? Chelsea rarely use one.
Left Corner: Eden Hazard
Right Corner: Juan Mata (Oscar sometimes)
Penalty Taker: Frank Lampard. If he's not on the pitch, then either of Hazard or David Luiz
Players To Go Up: David Luiz/Branislav Ivanovic/Gary Cahill/John Terry.
Long Free Kick: David Luiz

The team changes quite a lot and we have no real specialist in anything (David Luiz is more of a hit-and-miss at times). Chelsea love to throw everybody forward at corners, even when we're sitting in front comfortably.
I guess Cesar Azpilicueta has earned his spot as RB. Benitez/RDM have been using him there for a long time now so Ivanovic's name could be deleted. It'll probably change when John Terry comes back.
