He was soooo poor at EURO...
I expected him to be the leading CB for Austria. Their success in the qualifiers was greatly because of the balance and composure he gave to their D-Line; Hinteregger was more into meddling a little bit further up the pitch and providing some extra depth in offensive phase; the sidebacks (Klein, Fuchs) were more like wingbacks, supporting moderately at both offensive and defensive terms. But then Dragovic looked awful against Iceland and Portugal, even being sent off against the latter.

At Leverkusen he's been in highs and lows, showing his best and worst in just a few matches. I'm most likely to switch his DEF/RES values; his tackles (specially the standing ones) are fairly effective, and his RES is decent... I would set it at 80, maybe even to 79.

Regarding Form, I'm sure it has to be lower than 6. Not sure if 5 or 4 though.

He hasn't recovered from that poor performances at EURO 16; he's been a shadow of himself since then. Pretty erratic at Leverkusen, and his loan move to Leicester hasn't had the expected effect on his football career. It is very unlikely they will sign him on a permanent basis.

Set aside lower Form, he probably needs his speed stats and Mentality to be nerfed down a bit.

He's been awful through the qualifiers for Austria, really amateur level. Agree with those suggestions

His general level has been underwhelming and I would think it is likely a time to tack down the defense to greens. Many better defenders than him in the league and doesn't belong in this realm anymore. Shame that he has not been able to step up to the occasion

His form improved a bit in the second half of the season. He's not as good as he used to be be, but nowadays he looks less prone to nonsense mistakes.
This was probably because he's been demanded to play the ball in a much more simple way.

Today he had an awful game against Alemannia Aachen wich is a Regionalliga team, yeah sure they won 4-1 and Leverkusen won the game being completely superior but the few times Aachen attacked he was exposed again, honestly is also Leverkusen system, playing with 3-4-3 makes them very vulnerable. Since the last season he has been always caught out of position. His TW seems high 60's being generous.
