RWF/AMF: Foggia/Mauri
SS/CF: Zarate
CF: Pandev

More recently this looks more like...
SS/RWF: Zarate/Foggia
SS/CF: Pandev/Zarate
CF: Rocchi/Pandev

Rocchi is the captain and Ledesma is vice-captain.

Actually since Rocchi doesn't usually start, the captains armband is usually worn by either Ledesma or Siviglia

my suggestions:

GK: Muslera
LB: Kolarov
CB: Cribari / Rozehnal
CB: Siviglia
RB: Lichtsteiner
CMF: Ledesma
CMF: Brocchi
CMF: Matuzalem
RWF: Foggia / SS: Pandev
LWF: Zarate
SS: Pandev / CF: Rocchi

and attack something like this:

----Foggia--------------------------
--------------------------Zarate----
---------------Pandev--------------

when did they play like this?

IMO it was a 4-4-2 with Foggia on the left playing almost like WF:

--------Ledesma--Matuzalem--------
Brocchi-----------------------------
-----------------------------Foggia-
------------------------------------
----Pandev-----Zarate--------------

this is the formation I'm using. This is what they used against Samp on wednseday, no :?

you're right, this is what I have been using on my OF:

--------------Muslera--------------
-------Siviglia-------Cribari---------
-Lichtsteiner----------------Kolarov-
------------------------------------
-----------Ledesma--Matuzalem-----
----Brocchi-------------------------
----------------------------Foggia--
----Zarate-------------------------
----------------Pandev--------------

Make Brocchi a RCMF and then add "change sides" to the game plan, this way Foggia (LSMF) and Zarate (SS) switch sides.

ok the players are placed well, now we need to discuss about positions: I agree with Brocchi CMF, but I suggest Foggia & Zarate WF (or Zarate SS), and Pandev SS (he doesn't play as classic CF)

I don't feel like updating....so I'm going to put Ace in the 1st post

by now Ballardini used 2 system: a 4-3-1-2 (against Inter) or last year's 4-3-3. by now I think it's too early to make an update, to me it's better to wait a bit

about names, you can replace Ledesma with Baronio and Pandev with Rocchi, both of them were banned by President after they asked to be sold

With Delio Rossi Siviglia used to play right-center defence and Cribari or Rozehnal left-center. Now with Ballardini as coach Cribari is on the right-center and Siviglia left-center. Brocchi always on right midfield and Mauri on the left.
Zarate is movin on the whole attack as classic supportin' striker, but mostly on the left to cut to the center and shoot with his right (i.e. goals against roma, sampdoria in the italian cup final, elfsborg in europa league).

By now this is the ideal startin' eleven formation used by coach Ballardini. Alternatives on brackets:

GK Muslera (Bizzarri)
CB Cribari (Diakite)
CB Siviglia
SB Lichtsteiner (Scaloni)
SB Kolarov (Radu)
DM Baronio
CM Brocchi (Dabo)
CM Mauri (Eliseu)
AM Matuzalem (Meghni)
SS Zarate (Foggia)
CF Rocchi {c} (Cruz)

4-3-1-2 on the pitch, something like this:

--------------------Muslera
Lichtsteiner--Cribari--Siviglia-----Kolarov
--------------------Baronio
----------Brocchi--------------Mauri
------------------Matuzalem
-------------------------Zarate
-------------------Rocchi

But i think it's too early to set a definitive eleven. Cruz could be starter ahead of Rocchi and Foggia is worthy of bein' there i think. Diakite could play instead of Cribari.
Pandev will almost surely never play again with Lazio shirt but i'm not sure about Ledesma. he used to play on Baronio spot in front of the defence line.
And let's not forget Davide Ballardini likes to change formation, so i think he won't always stick on 4-3-1-2 like now. He also tried 4-4-2 (Eliseu, Del Nero and Foggia are natural wings), 4-3-3 and 4-2-3-1 on preseason. Only sure thing will be four men on defence line.

Rocchi is the captain. If he's not playin' is Siviglia. With Siviglia injured third choice has been Baronio last match, against Atalanta.

Zarate takes penalties and direct short free kicks. Kolarov takes direct kick from bigger distance. Baronio is on duty for undirect free kicks and for corner kicks (sometimes also Zarate with Baronio waitin for a shoot oportunity at the penalty area limit).
